Athletics Information Packet
Athletic Packets for CCSD are now completed through Aktivate.com (formally known as “Register My Athlete”). Parents, please create an account and complete the athletic packet. The CCSD Physical documents are also available for download. The CCSD Physical form is the only physical form allowed.
